Reablement means short term care at home is to aid recovery after discharge from hospital which is more specific and helps the individual to get back on their feet.
Post Operative Care and Post Discharge Care
Our Discharge Coordinators/Liaision Social Worker will facilitate smooth transfer of care to the home/community from hospital
Regular visits by Social Workers/Nurses and provide the care required for the client, following a hospital admission, surgery and subsequent discharge from hospital
Our Liaison staff will provide the necessary feedbacks to the family on a regular basis - Peace of mind and less anxiety about your dear and near ones who are under our care
