Virtual Wards offer patients the care they need at home, instead of in hospital.

Medically stable patients are transferred home into a Virtual Ward, but remain a hospital patient, Their hospital consultant agrees and monitors the treatment until it is completed.

Clinical home care is provided by our highly skilled Multidisciplinary teams, supported by technology, delivering care at the right time in the right place.

Remote patient monitoring (Telehealth) provides the option to review the patient’s vital signs between our visits to their home. This can provide additional reassurance to the hospital team and patient, as well as highlighting changes in the patient’s condition. 
Passing away peacefully, with dignity and within familiar surroundings, is what many of us would wish for. End-of-life care at home allows you, or a loved one, to do just this, in the place you feel most comfortable and safe.
